Students at St. Peter’s Middle School and McAuley Catholic High School can receive their flu shot. Fill out the attached, and return it to the school office. If you include your health insurance information, then the flu shot will be filed under insurance. If you don’t submit insurance, please bring a check made out to The Medicine Shoppe for $25. Download the Immunization form.
The flu shot will be given the week of November 4, 2019.
